Oscar Taveras (RIP) could've lived up to his #1 MLB prospect potential if not for his fatal crash.
Grady Sizemore could've been a LH Trout if not for injuries.I know there are many more, those 3 just came to mind

Honestly the best example right now is Byron Buxton. A true 5 tool player but made of paper mache apparently.
