Substitute a RP for your outfielder. Swap Positions with that OF. Ohtani is now in the outfield and you have a pitcher pitching. They used to start with no Stamina and I've never tried this year so I don't know if they still do. Your other option is to swap Ohtani with the outfielder you want to take out of the game. However, with the knew pitching rules, this OF will now have to pitch to three batters. Probably with 0 Stamina. I'd rather have an actual pitcher out there with 0 stamina than a position player. A double switch won't work because a double switch is used to leave the player that pinch hit for the pitcher in the game. You're not pinch hitting for the pitcher.

For the Show 20 in DD you can not have him both pitch and play the field in the same game, because it will not let you swap positions with the pitcher. You can in other modes.

This isnt a complete answer but it answers one of the problems. When you pinch hit Ohtani, you have to warm up another pitcher AND "schedule to put in game" before the half inning is up. Otherwise you'll have a tired Ohtani facing 3 batters the next inning
