Online H2H Issues (Field Throwing, Baserunning)

I just started playing ranked online H2H and I've noticed a few issues that I don't know if they are lag, if I suck, or if it's the game itself.

1 - I will have an easy throw out at 1st base, but my infielders will just hold the ball. They will make the throwing motion as if to fake out the runner, but won't actually throw it. They will make the fake attempt multiple times and the runner will be safe. This has happened in multiple games now.

2 - Trying to get my baserunners to advance to an extra base doesn't seem to consistently work either. I had a clear double and my runner stopped at first. I kept pushing the button to advance him to 2nd and nothing. Once the ball made it to the infield, then the runner took off all on his own and was thrown out.

3 - Pitching Relief - Ever have an issue where you warm up all your guys the right way, but then they come in and can't hit the strike zone to save their life? I have a great game going into the 7th, brought in my bullpen guys, and just got blown out. If I didn't leave the icon in the center of the plate, the I would throw a ball every time. This was for 3 straight pitchers.

I can only help with #1. Are you having this issue on ALL throws, or just some throws? I sometimes will hold onto the button when trying to turn a double play and the relay throw never happens. If that is what happens, try not to hold the button when still chasing after the ball until just as you get it. If it isn't that then I won't be of much help here.
