I was expecting something hard hardcore or racist etc, was rather disappointed. Also negligence? Stuff is going to fall through the cracks all the time, no censor is going to perform at 100%, just report it and move on

Report it if it breaks the rules and put it behind you.

While I agree that people get too offended too easily I also think players who create these questionable names should then take responsibility for that choice if someone does report them.
In the end though it is Sony's or SDS's call whether a name violates their policies.

Context matters, if someone gets offended over someone’s name being something like poopyhead69 that’s stupid. (Not a real name I’ve encountered but don’t doubt someone has it). Plenty of people have legitimately racist or just straight up wrong names that should be reported. The people who complain about every tiny little thing tho take away from the legit complains.

I can't remember exactly how to see OP's game history, but I had a situation like you described. I played a guy in Ranked and his CAP (wish they were banned from online) name would be offensive to some, but childish to most.
I thought about it and decided it wasn't bad or racist or anything blatantly awful, so I just let it go. If it's offensive enough to report, then OP should report and not say anything. Less likely someone will retaliate against him.
