I had button accuracy and still had errors. They went away when I changed to Buttons.

Without button accuracy, you are subject to accuracy rating of the fielder and RNG. If you hit the green with button accuracy, it’s impossible to make a throwing error

Like said below, button accuracy is the only way YOU can control the actual throw. Buttons only will default to your players rating. Practice with button accuracy it’ll help you in the long run.
