More commentary lines, and guest commentators like in NBA 2k... Have a few players be guests for a game..
In Japanese Spirits baseball you can choose 9 different combinations of broadcast teams...

I wanna see foul balls off the catchers knock their masks off, or off someones shin or ankle see them walk it off, or bring a trainer out... See someone sprint to 1st base and maybe pull a hamstring, not too grahpic but maybe they hit the bag awkward and roll an ankle....

Games at dusk players losing the ball in day to night transition...

I would love different commentators. The catchers mask being knocked off is probably something the MLB won't allow them to put in the game just like charging the mound or concussions in Madden. Also while I would appreciate the realism, there is enough wrong with fielding that we dont need to throw another factor into it.
