Throw the ball in from the outfield or infield and get the out on the runner going home. It's that simple

I actually completed the trophy in a player locked moment with the weird show time throw interface during a MTO run.
Otherwise just have strong accurate throwers in the outfield and get lucky enough to throw someone out. Helps to just spend some time practicing throws using this interface. I screwed around in practice mode until I got a hang of the meter.
