How can I get certain unlockables?

I was scrolling through unlockables and equipment when I saw that the ones that aren't on the market have "discovered by" text at the top of the screen. I'm assuming most of, if not all of these come from program rewards. I was wondering if anyone knew exactly what I had to do for specifically the "Just like the ex girlfriend" home run call. Any help is appreciated!

Usually they are Rewards for doing things like 12-0 in DD, WS title, Conquest and such

...some are hidden in conquest maps.
