Why do pick off throws randomly have a throw meter once in a while?

I can’t tell you how many times I’ve punished cheesy base running only for my pickoff throw to sail because there’s a throw meter on about 25% of throws. And the camera only changes perspective after the ideal release point.
So technically there is HIDDEN throw meter that SOMETIMES triggers that doesn’t reveal itself until after a literal jump cut. What am I missing?
It’s already hard enough to manage base runners with infielders missing tags and no bad jumps. But having the only direct counter to stealing (slide step, pitch out=indirect) be an unreliable gamble neuters defenses to the mercy of base runners.
Each time it happens I have to stop playing for a week or so in fear of reverting to my 12 year old hulk-smash form.

It's because if they take a 3-step lead or more, the runner automatically runs to the desired base they want to steal, thus starting a pickle. It's hard to explain, but it's mainly because of the 3-step lead or more.

You need to just “step off” with L1, don’t do the pick off. Then you can just throw to first , or to second if they were going. That’s the fix

So the meter triggers as soon as runners start to advance, and doesn’t trigger when the runner retreats. But that means the que to alert player of advancing runner (perspective change) happens AFTER the meter actually begins. So the first time the player actually sees the meter in fielding perspective, the meter is already red.
So technically, the player has already made an error even before he’s qued, by camera change, that a meter has triggered.

Those guys know what they are doing and are trying to bait that bad throw. Another cheese ball move, so just step off or change your throwing meter once you know they do it and you will pick them off every time.
