Keeps happening to me too although I may have finally found the fix as it worked for me just now. You have to clear the cache and delete your saved file so it reloads.
• PS5: Boot in safe mode > “Rebuild Database”
• Xbox Series X/S: Hold power for 10 seconds > unplug 2 mins > boot
• Switch: Power off > hold Volume Up + Down + Power to enter maintenance mode
