How do you add an icon for your diamond dynasty team so you dont just have an empty square where the logo/icon should be

So i just took a loss in an extra inning game when my pitcher did not throw one pitch in the last inning. Tied 0-0 going into the 14th and my pitcher is pulled due to out of energy…..did not throw one pitch that inning. New pitcher comes in and gives up 3 runs, but my pitcher is credited with the loss, but no affect to hus ERA. How does that work? Are they just saying he is automatically responsible for the guy on 2nd that starts the inning there? Unreal and stupid if that is the case

Which makes no sense at all. Most people play to get their player to 99. Othani is 99 on the game and is a two-way player, so why cant my guy be 99 as well???

I have a SP/RF in RTTS and fully maxed out all categories but only a 90 for the RF and 94 for the SP. have all diamond equipment and all diamond loadouts. Is there a way to get to the 99 rating? Am i missing something or is this a glitch?
