I assume it because they maybe don't count a 2 way RTTS players for this Trophy or maybe you have a RP...
Either way the easiest way to pop this Trophy is to load into an away exhibition game (on an easy difficulty) with Ohtani leading off and Hit a HR in the 1st AB of the game, if you dont hit a HR just pause and restart the game until completing.
