Extreme lighting during night games and the pitcher just stares at the catcher in RTTS while balls are in play behind them.

Fielding. It's just stupid.

Pitching. It's a crapshoot.

Game breaking bugs. Ground rule home runs. Your opponent suddenly having empty bases and three runs after a passed ball. I hit a walk off homer the other day and the game didn't count the run and it ended as a friendly quit. Game breaking in the sense that it defies its own rules.
