I know how to use him only as a dh, and I know how to use him only as a pitcher. But how to I use him as both in the same game? I tried to sub him in at DH when he was pitching and it wouldn’t let me swing or move PCI until after his AB. Then once I tried to bring in a reliever it forced the reliever into the DH spot. Is there a way to double switch or something to prevent these issues? Also does it matter CP or SP Ohtani?

It has to be SP Ohtani and you just have the same card in your rotation and in the DH slot. Set this is your lineup screen

You don't do it in game, switching the DH in game forfeits the DH which is why the rp had to bat.
On the main lineup screen you put him in at both spots, pitcher and DH.
also the CP version can't DH, all other Ohtani's can

Ok I understand. For the record though you can use the CP Ohtani as a dh as long as he is not on the mound. As long as you sub in after game starts

You can, but you lose him as an option for pitcher. CP Ohtani can't start the game as a DH, nor can he pitch and hit in the same game.
