The portal is open trophy PS5

I’m trying to double platinum the game and create a diamond dynasty team doesn’t pop up on my PS5 anyone know how I can fix this?

I've discussed this with a PSN friend.
It's impossible apparently unless SDS fixes it so you can create another DD team.

I am going to test this on my alt account later because it’s risky.
if you’ve popped the trophy on PS4 or PS5 already, you should be able to unlink your console account from TheShow website and launch the game on the console you want to pop the trophy on. With your account no longer linked it should prompt you to create a new DD team and pop the trophy. After this I would re-link my account to get my old DD data back.
It should work but I’ll let you know

Unless I'm misunderstanding what you're staying, there is no account linking between PS4 and PS5 to do this. It's just one PSN account.

Could you please share what you did? I played it on PS5, got a Platinum and am now trying on PS4 but I can't get it to reset.

I am on the Xbox and switch and started the team on my switch. If I unlink my Xbox I can relink correct?

Go to your account in game- select unlink account.
Log onto the show site- click baseball icon make sure only the show account is active.
Back in game select to link- get the password
On the show app- go to baseball icon- scroll to bottom of the list and there is a box to enter the cross plat code or whatever- enter it. It will then ask which account you want to link- pick the new one with lvl 0.
Back in game- load up the DD, get achieve. Gonna to account in game- unlink
Back to app- make sure unlinked-
In game get a new code to link
In app baseball Icon scroll to bottom of accounts- put in new link code- now choose your old account. Link it.
Start game- you’re GtG.
