I only saw this in that moment. It didn't appear in any of the others. It may be fixed to just that moment and nowhere else.

It appears so.
It's technically a glitch; the scans get deleted from the server, and the art team just doesn't patch or replace them. I really hope new artists are brought in for 26 because this team has been brutal with polish, visual quality, and assurance.
