Game wont recognize button press at pinch hit screen

Was just beating someone by 1 run in the 7th inning. My pitcher was coming up and it switched to the blue screen asking if I wanted to pinch hit for my pitcher. I was pressing A and the back paddle which is mapped to the A button and it wouldn't register the button press. The timer ran down and dude took the win with my game being forfeited.
How TF does this happen? I've been playing since 2014 and this is the first time I've seen this. I unplugged the controller, it's a wired Power A and plugged it back in but it wouldn't recognize me pressing the A button. I hit the home button and that worked, went to settings and pressed A and it worked. Is this some kind of forced glitch the other person did? -

There's just a bunch of bugs in the game that we have to deal with. I've had times where my pitcher won't throw the pitch for whatever reason and my time ran out throwing a fastball right down the middle of the plate for a no doubter. I've had games freeze completely and have to quit out for a loss. I've had my first baseman field the ball and just freeze not able to move or throw giving up a hit instead of easy out. Logos disappearing since launch. I just expect bugs to happen... especially in H2H matches and less frustrating realizing they are going to happen. The game has a lot of flaws and just to accept this game for what it is... even though it's super frustrating to lose a game because of a bug.
