Things I would like to see changed...

Overall I love MLBTS, but there are some things I would like to see changed:
1.) Get rid of the outside PCI: The outside PCI in my opinion is the #1 factor to randomness in hitting results. Give me the ability to turn the outside PCI off and increase the inner PCI by 10% or something like that. I want to swing and miss if I miss, but I want better results on good- good timed swing inside the PCI.
2.) Baserunning Indicator: I want to know earlier when my player is planning to go to the base after my next base. Instead we get an eventual wide turn with no easy means to correct it and your opponent throws back for the out. This happens to me fairly frequently and is very annoying.
3.) Pitcher Confidence: A pitcher being confident leading to a shrunken PCI is insane. Give them better placement on pitches for sure, but the pitchers confidence has nothing to do with the hitter confidence. Not sure why this is even a thing. H/9 can still be used to limit the hitters PCI, but confidence should have nothing to do with it. If you are going to allow pitchers confidence there should be a hitters confidence that limits a pitchers ability to throw strikes.
4.) Base Stealing: If the pitchers is losing stamina on throw overs the baserunner should lose speed based on being more tired from diving back into the bag.
5.) Perfect-Perfect rewards: They said the perfect-perfect hit to out ratio was high, I am out more than 8/10 on perfect perfect hits.
6.) Turn off shifts: If people want to talk about skill, turn auto shifts off. Let the player use baseball IQ to shift if warranted.
7.) Overhaul to pitcher stamina: Using PPP deGrom can throw 120 pitches and still not be in the red, but my top reliver comes in and after 6-8 pitches the stamina is fading fast at times.
8.) Fielder responsiveness: Especially at first base. A routing double play when runners are at first and second should not be penalized by letting that run come home from second.
9.) Cutoff throws: Fielders that cutoff the ball are often too close to the infield, have their backs turned to the field for no reason, and jump to cut the ball off when they do not need to. Take away some of the crazy behind the back animations and allow guys to make routing cutoffs look a little more routine for elite levels fielders. Also let the cutoff men travel a bit deeper into the outfield to get cutoffs when needed. If my outfielder a a 76 arm, but my infielder has a 98 arm for example... let my infielder make a better intermediate throw.

I agree with everything but your 5th point.
"I am out more than 8/10 on perfect perfect hits." is simply not true. Your mind just remembers the outs more because it feels out of place. If you actually tracked it you would see the actual stats.
For example, in the last 10 games you've played that had perfects in them, the results was 19 perfects, 15 hits.

I agree with these too except 4. Why kill my runners stamina because they won't stop trying to pick off? I was playing a stupid CPU game and I had Honus on 2nd base. The CPU threw over 3 times in a row... I took 1 step and they ended up trying to pick me off 10 times. I was winning like 42-0. The game's logic is broken. Well, the entire game is broken.

I am talking about online with secondary leads. Throwing over actually deters the runner in real baseball. It is kind of funny that against the computer my guys often get [censored] jumps (no the steal method), but online guys just take off.

I only counted online games.
And yes, your statement is wrong. You said 80% of perfects were outs. Unless I was misreading it, but the way you phrased it up there makes it seem like you are "Out more then 8/10 on perfect/perfect hits"

- I’ve literally seen the game move a shift I put on during an AB. Game was tied in an event game. Saw them in, changed pitchers, came back and they were moved back.
And I’ve tested the theory before, changed pitchers mid AB and my fielders didn’t move. Magically in the game above, it cost me a win.

Can't you turn off the shift in the Gameplay Settings?
Or do you mean having it set where there is no shift unless the player selects one?

#2 can be completely avoided by using classic baserunning (buttons and d-pad).

I just meant less than 80%, not that I was only safe 20%.
