Freeze off forfeit, can someone explain?

So this has now happened to me multiple times and I don't understand what's going on.

Game freezes, so a freeze of situation and I wait, but then after awhile screen pops up says I forfeit.

I'm in the 700's now trying to advance further and I keep running into BS. Freeze offs, slow play, and all sorts of nonsense from players trying to scum a win.

Also how does the in game report system work? I havn't been able to find it an dthe PS one just refers back to the game.
