Most likely the latency, where the servers have already registered the ball in the glove of the 1st baseman but that info is delayed in your screen giving you the optical illusion that he actually might have been safe.

No problem. For this same reason it's harder for people who have latency related issues to catch up to high velo fastballs; in their screen the baseball is not in the "swing window" yet but in real time and according to the servers the ball is already going through the strike zone. Which will result in a late/too late swing every single time.

Not that I know of, if you don't have latency issues with other stuff that is. So if your ping is lower than 30 for other games and applications, you should be good and there's not much you can do about it. It also depends on SDS servers and the location and internet quality of your opponent.
So for myself for example, I split my time between Amsterdam and San Antonio, TX. Whenever I'm at home in Amsterdam, I have more latency related issues compared to when I'm in Texas because in Texas I'm closer to the servers and most of the game's population.

Last year I sent a video of a play that cost me the game. Ball bounced a foot in foul territory and it was called a fait ball.
