Your post has also caused me to think about the way that I hold and use the controller. It is very possible that I am accidentally and unknowingly clicking the R1 button when I make contact with the ball. I sort of squeeze the controller, press the X button too hard, and even move my hands upward when I "swing". (Yes, I understand that my excessive thumb pressure and erratic hand movement is ineffective and unnecessary, but I just can't help myself. I may need an intervention.)
I will report back after I get a chance to try out this solution because it might also help others if it solves the problem.

That is so weird; never heard of this bug before. The only times it happens to me is if a liner is hit towards SS and there's a runner on 2nd; he'll dive back and at best depending on speed he'll get to 3rd when he should have scored. But runner on 1st? Don't remember seeing that. Sucks.

@Orcin52_PSN If it's not your triggers...I would bet it's what you're thinking. You're hitting R1 unknowingly

SOLVED!!!
I was unknowingly clicking R1 with my right index finger when I pressed the X button. Of course, it only mattered with a runner on base. I played a couple of conquest games while consciously holding my right index finger away from the R1 button - and the baserunning was correct every single time!
Now, I just have to get comfortable with a grip that doesn't wrap around the bumper button, which shouldn't be too difficult. For anyone else with this or a similar problem, make sure you aren't touching the bumper (R1) or trigger (R2) which stops the runner. I have seen people complaining about the runner "freezing" on the basepath and an accidental trigger click could be their issue.
