SP confidence

Does anyone know what determines how much confidence in his individual pitches a starting pitcher opens the game with?
Obviously, warming up your bullpen guys before bringing them in gets their confidence up and there is the 9 pitches warmup option as well.
Now starters, on the other hand, seem to enter the game with really low confidence in all pitches and need to develop it by throwing strikes and getting outs.
I don’t get it though. Is this unique to conquest? Is it because the first inning is like the 7th inning in effect?

Are you bringing in starters from the bullpen?

No, I’m talking about the very beginning of a conquest game. I’m trying to understand why my SP comes into the first inning with low confidence in all his pitches and has to gain it by throwing strikes and getting outs.
Relievers, assuming they get warm first, enter with much better confidence.
