Or are catchers not throwing like they should be to throw out runners in RS again? Holding the ball way to long when pressing triangle to throw.

It sometimes helps if you aim the left stick towards the 1st baseman and then throw the ball. Somehow the game adds urgency when you do that, but it doesn't work all the time. Try it out next time and you'll see what I mean.

Catchers in general have no sense of urgency.....with one exception.

Pop a ball up above the plate, and my catcher is up and under it at the speed of light.......
Swing a little dribbler 6 inches from the plate and Miguel Cabrera will beat out the throw.
