Yes this happens to me it just cost me a game as it completely ruined my mood and focus. It happens a lot but usually my fielder gets up and makes the easy fly out.

It's a controller issue, my outfield and infielders do it when i use my backup controler (it has seen some anger from moments) my other controller is in great condition and dosn't have any issues like that though.

Get a new controller. I had same issue until I bought a new one
