Check Swings by Accident for a Backwards K

Been grinding conquest against the CPU. I was batting against Zach Plesac with Future Stars Austin Riley when he throws me a cookie high and down Main Street, on a 3-2 count. I begin to swing...
...and Riley stops. The ball lands right there in the middle, and I go down looking with him. What. The hell.
This has happened to me multiple times against the CPU. It happened to me with Grady Sizemore a few games ago, with the same pitch—a cookie down the middle to go down looking.
I'm starting to wonder if this is an in-game issue, a controller issue, or a motor skills issue. It needs to stop—striking out on a pitch that I was going to hit in that manner is very costly.
This happening to anyone else?

Yes I have noticed on several occasions where I push power button swing normally and it registers as a checked swing. And yes this has occurred when the pitches were legitimate strikes and I had good timing in the swing. Sucks but I chalk it up to me just pressing the button not as hard as usual. Maybe it is a programming error… I dunno. What I do know is the checked swing mechanics are broken anyhow and need revamped.

It only happens to me on meatballs down the middle with dead center pci and good timing. Ohh but if I ever so lightly tap X on a chased pitch, it's a swing 100%

I often times will be batting and my pci jumps around all over the place when I move it. I'll go to swing and the ball will be back on its way to the pitcher before i swing. (Being facetious, but you get my point). And twice...TWICE!!! I've gotten to the last out of a perfect game to have a fly ball hit to my centerfielder, and he's dead in his tracks. Will not move.
I have a fairly old controller so I believe it's losing bluetooth connection. Not saying our experiences are the same, but it's a possibility.
